The surgery has started to provide a full travel clinic service.

A Travel Risk Assessment Form must be printed, completed and handed to Reception 6 weeks prior to travel. The form will be reviewed by one of our two Travel Vaccination trained Nurses to review and they will contact you to discuss the vaccines recommended and to inform you of the cost.

Please note we are unlikely to be able to assist with last minute travel vaccination requests as we allow time to do this safely and appointments are during allocated clinic slots.
