Requests for repeat medication can be made by returning the tear off slip attached to your last prescription or online by using the "Patient Access " page via the above link.
Please note that requests for items not on repeat must be made in writing to your doctor. There is a space for this on the patient access prescription page.
All prescriptions are available for collection 72 hours (3 full working days) after the request. Please allow extra time over weekends and Bank Holidays when the surgery is not open.
Prescriptions can be collected from Shalford, Bramley or Wonersh Pharmacy.
If you run out of your regular repeat medication during the out-of-hours period contact your local pharmacy.
EPS- Electronic Prescription Service
The Electronic Prescription Service (EPS) is an NHS service. It gives you the chance to change how your GP sends your prescription to the place you choose to get your medicines or appliances from.
The service enables us to send your repeat prescriptions to the pharmacy of your choice, where you live, work or shop.
To use EPS you need to choose a place for the surgery to electronically send your prescription to. This is called a 'nomination'. You can arrange this by filling in a form at your pharmacy or in the surgery.
Should you wish to change your nomination or cancel the service, you can do this at any time. Please advise your pharmacy or the surgery before your next prescription is due.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.
Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
